You were attempting to install the user authorization using the Forced Install method. The Reset Code you typed was invalid, so the user authorization did not install. Repeated attempts to install with an invalid reset code will permanently disable your disks. You can only obtain a valid reset code from your technical support representative. Recheck all the codes you and your representative are reading to each other and try again.
